    Re:  Benefit Coverage for Participating  MembersParticipating Members of the Canadian Association of Journalists

     

    Dear CAJ Member:

     

    Because many of our members have asked whether a health benefit package could be arranged through the CAJ, we contacted a benefit administrator to determine our group’s eligibility.  We entered into discussions with Global Benefit Plan Consultants Inc. which has resulted in a package being underwritten by Liberty Mutual.

    This package includes many of benefits  webenefits we were seeking for CAJ members at what we believe is an affordable and competitive price.

    Those benefits include (see SCHEDULE OF BENEFITS - (PDF - 59K in size) for details) partial coverage for: drugs, dental, vision and hearing care,  health, health practitioners,  foot, foot care, out of hospital benefits, etc.

     

    Since  theSince the CAJ has no past record  asrecord as a group insurer, we were not able to   obtain a plan at this time that included coverage for accidental death and dismemberment  ordismemberment or short and long  termlong term disability.  We hope to be able to  includeto   thoseinclude those in  ain   futurea future policy  packagepolicy package once we have a history with Liberty Mutual.

     

    If you wish to participate in the proposed plan, please complete and sign the BENEFIT REQUEST FORM - (PDF - 47K in size).  

    This form is to be returned, along with your initial payment cheque (payable to Liberty Health)

    Payment will commence following your approval by Liberty Health.  If, for any reason, Liberty Health does not approve your application, your initial payment cheque will be returned to you.  Please note there are two categories of plan for you to choose from:

     

    a)  Guaranteed - Base Plan - refers to members who supply no medical evidence;

     

    b)  Non-Guaranteed - Silver Plan - Rrefers to members who have provided medical evidence and been approved.
